Practice Charter
Confidentiality
We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act.
The practice adheres to the NHS Caldicott Guidelines which dictates that staff members are only allowed to access patient information that is directly relevant and necessary to perform their duties.
Freedom of Information
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available.
This scheme is available from reception and can also be viewed on this website.
Access to Medical Records
Patients registered with Dr A V Lee & Partners can request access to their own medical records held at the practice. Please put your request in writing for the attention of the practice manager stating whether you wish to view your records or obtain a copy. There will be a charge for providing you with a copy of your medical records. Please enquire at time of application.
Patients are advised that if a third party such as an insurance company or solicitor wishes to see their medical record to support an insurance application or an accident claim, then the patient will be asked to give written consent for access to their NHS record. It is important to realise that such consent will relate to the whole life-long record, unless otherwise stated.
Complaints
The practice has a formal complaints procedure. Should you wish to make a complaint, please either pick up a copy of our complaints procedure and policy from reception or write to us with your complaint addressed to the practice manager. Normally a written reply will be given within 10 working days. There is a right of appeal to the Complaints Department at NHS Leeds if the complainant still feels dissatisfied with the result.
