Obtain A Repeat Prescription?

If you take medication on a long-term basis and do not always need to see the general practitioner for your prescription of medicines, you will be issued with a computer slip attached to your prescription. This should be returned to the practice to request further medication. You can either post the repeat order form or hand it in to your surgery's reception. You can also order your prescription via e-mail using the form below.

Please allow two working days after your repeat prescription request is received by the practice before collection. If you wish for your prescription to be sent to the local chemist please inform the staff or write the chemist's name on the repeat order form. Please note that only in rare circumstances will requests for repeat prescriptions be taken over the phone.
